Optometrist -(Sunday Only; Meals and 4 Hours CE Included) – $150.00
Student - (All Days; CE/Meals Included) – $20.00

TO RECEIVE EARLY REGISTRATION RATE: Forms and payment must be received on or before 12/21/18. CANCELLATION POLICY: All cancellations must be in writing. In order to receive a refund of 100% of the registration fee (less a $25 cancellation fee), cancellations must be received prior to 1/1/19. No refunds after that date. Please note that pre-registration closes on 1/9/19. Registration can be done onsite after this date.
